Statement of Patient Responsibility
Provision of Information:
Patients have the responsibility to provide complete and accurate information about their health status for an accurate assessment and appropriate treatment.
Compliance:
Patients may be asked to sign and comply with a medication contract. Patients are also responsible for keeping scheduled appointments and for notifying Colorado Pain Specialists when unable to do so.
Refusal of Treatment:
The patient is responsible for the consequences if they refuse treatment or do not follow program instructions.
Fees/Charges:
The patient (or guardian, if the patient is a minor) is responsible for assuring that the financial obligations of his/her care are fulfilled.
Respect and Consideration:
The patient is responsible for being considerate of the rights of other patients and Colorado Pain Specialists personnel.
